Don't know how it is possible.
Requirment for Thick Skin lvl4 is 41 Con...

You can see that your effective Con right now is 12. Got any debuffs on you? Diseases, maybe?

No, the effective Con there is 44, but Thick Skin requires 50 for level 4.
