Yeah, I agree the patience is the greatest virtue in ToME. But knowledge what's out there and open-mindedness to diving are also very important.
My receipt would be:
slowly prepare -> dive where you can get better -> slowly prepare for the next dive -> dive... and so on until you're ready for everything, then it's only diving left to do and win.

It's a good thing to remember that if you leave one unique monster alive, you might find it later somewhere else. Same goes for un-id'd artifacts if you have preserve mode on.
I have a bad habit of trying to find every artifact on "Superb feeling" levels and kill every unique I see. That's what usually gets me killed.
For example a swordmaster who got tired of easy dungeons and went straight to Angband at clevel 31. He killed some great quest wyrms quite easily, but when he saw Dreadmaster Tselakus at level 69 along with two ringwraith I just had to try even while I knew he's gonna die. To my amazement he managed to kill Tselakus, but since he didn't have any phase door, teleport or healing available he couldn't flee from the ringwraiths.

I only tend to play warrior types at the minute, determined to win with one of those before going for what I feel is a more complex char. Unless I have a very bad game, where not much goes right, I find I can get a swordmaster up to level 20, 30 most times.
Where I usually go wrong is in rushing into quests I'm not ready for. Last game I played, I died quite horribly in Gondolin around level 32. Won't say more, being as this isn't the spoilers forum, but the char dump is here for anyone interested:

I agree with all the advice above. Take it slow, be VERY sure about what you're capable of killing. Tread carefully, watch out for traps. Run like hell if it goes wrong. And keep lots of methods of running away handy.

That's what I do too. Char that doesn't rely only on spells. I don't play ToME too often, so I don't know how the magic system really works now (it changed much over the years). Easier with monk/specialized-fighters/possessors. The goal is to have fun right ? Not trying to know everything you can... Well, fighters are easy, possessessor is easy after you get decent body, but monks. Monks are hard. At first it seems easy but after special/elemental attacks kick in it's hard.Easier with monk/specialized-fighters/possessors.
My winners (and all almost-winners) have been either warriors of possessors. Spellcasters (mainly sorcerors, because I'm not so familiar with spell schools), rogues and other loremasters than possessor don't live that long with my playing style.

My only winners so far have been Geomancers.
Actually, I'm surprised the class was chosen for the challenge, since it's not exactly "challenging" once you know what you want. Geomancy is like a mini-sorcery skill, and in the end you get to have most of the spells you would have as a sorceror without the hp problem.

There's no "trick" for winning the game. I don't even think that there's a golden rule to live by. It's just a combination of strategy, experience, and luck.
Just learn from your mistakes. Rethink, recharge, return stronger than before (my often misquoted motto). To that I would just add:
*Know yourself. Know how much damage your character can take, know which spells you can and can't rely on, know your weaknesses.
*Know your enemies. Figure out which ones are dangerous and which can be fought without risk. Figure out which ones are worth the XP and which ones are just a pain. If it's too tough, don't fight it!
*Be prepared. Healing, teleportation, genocide, undead form, blood of life, spellbinder, resistances, whatever helps you to stave off death.
*Always fight enemies on your own terms. I think that this is the most important one of all. If you're stuck in a room with fifteen Inertia Hounds and a Great Swamp Wyrm, don't just hack at the hounds until you die. Read a scroll of Phase Door, find a nice corridor, fight them one at a time. Fighting a big summoner? Fill the corridor up with runes and lure it to you. A lot of deaths are caused when you let the monsters rather than you control the battle. Sure, you'll still die sometimes, when that offscreen Grand Master Mystic summons twenty Aranea and they all cast curses on you at the same time, but as a general rule, if you're in command of the battle and know what you're doing, you won't get killed nearly as much.

There are still occaations when you enter a room and have that light radius 5. If the rooms is bigger and you don't light it immediately, there might be that GM mystiic lurking in the shadows. Of cource this can be prevented with enlightenments and sense monsters and stuff, but they're not available every time.
It has happened to me that there is a monster somewhere around the corner so that it can see me and summon/breath while I can't see it.

I think that the advice about the Maggot mushrooms begs repeating.
Don't eat them when you re at Full. Just don't. I just lost a lvl 15 hobbit slinger to a bunch of Black Orcs, because I thought I could take them... I probably would have, if I didn't get gorged. Bye bye Nimthanc...

If you feel lucky, be darned careful. Try not to make stupid mistakes - finding a powerful artifact is often followed by finding a nasty out-of-depth monster. Keep a clear head and pay attention, even if you've just found Ringil on level 10.
Don't, for Eru's sake, play a yeek, unless you really know what you're doing.
If you have a low-medium level mage (any type), try to keep opponents at arms length. Remember, melee is your weakness. Don't try to melee tough opponents until you're at a high level, when EQ, potions, and skill boosts have given you halfway-decent melee power. If you're a sorc, try not to use melee at all.
If you're a warrior, don't take on monsters with powerful melee until you're ready. A warrior generally doesn't have good ranged attacks, and so will probably have trouble with opponents sporting vast melee power.
Always remember what your god likes and what he/she doesn't like, and how to take advantage of those likes/dislikes. Being in the favor of such a powerful being as a Vala can help you a lot.
