Ghasts and ghouls like to spam retch everywhere which can damage and turn neutrals hostile. Imagine standing next to Linaniil and have your ghoul puke all over her. Noice. Papa gets to make a new char now.
Would probably be best to add a check in their AI to make sure they don't puke in towns unless hostiles are in sight.
