So I'd been testing through most of the 1.6 beta, but when the full release (and accompanying expansion updates) got pushed, a new issue cropped up. Suddenly, the tinker cave isn't actually appearing on the map when that option is selected from the escort. Initially encountered the issue using only official dlc/addons, but used the select-your-escort addon to reproduce the problem. The cave appears properly when starting a tinker character, but it no longer seems to appear with any other characters who take the escort.
On the off chance it's a contributing factor, the initial bug observed and all (maybe? at least nearly all) the of the reproductions I did involved the tinker escort being encountered in the first dungeon, before the character had explored the world map for the first time.

As a question/possible other bug-- is there supposed to be rhyme or reason to when/where the APE drops in this build? during the beta I had characters who got the APE but no tinker escorts and others who unlocked the tinker tree but had no APE to make use of it. Cant tell if that issue carried to the full release with the above issue being what it is, but thought it was worth mentioning.

I got a tinker escort with 1.6+DLC installed. I found the tinker cave but it wasn't in the spot that it used to be. Not sure if it changes places like other landmarks.

1.6 changes the location of the cave to a random location.
I was able to reproduce the issue with only the official dlcs enabled too (though with the developer mode enabled to speed up testing).
I was also able to find the tinker cave once while using my select-your-escorts addon so I don't think it's directly related, but it guarantees you'll find the escort quickly.

I just tested going to the world map first before encountering the tinker escort:
1. Starting as a Cornac Berserker (so starting in Trollmire)
2. With the select your escort addon enabled so the Tinker appears first
3a. Going back into Trollmire and finding the escort: 0/5 times the cave appeared
3b. Going outside to Kor'Pul or Old Forest to find the first escort: 0/3 times the cave appeared
Also tried having the tinker be the 2nd escort once, didn't appear.
So I don't think it matters whether you go out to the world map.
Also tried starting as a Temporal warden, didn't appear.
I'm thinking it may be entirely random whether it works... maybe based on where it tried to put the cave.

Could it be that Tinker cave spawned on Far East?

I was checking the for the location using the te4 log. It didn't appear.

Ok I think I figured it out...
It is trying to find a free random adjacent spot based on the x,y coordinates that your player was at INSIDE the zone before you leave to the world map. In many maps, is at the edge of the map if you take the exit, so that ends up being 0,y or x,0... so it tries to find a spot near the edge of the world map, which has no free spots. [when I was testing, I was mostly using trollmire, which has the exit at the left side of the map]
If you use the rod of recall, your x,y won't be near the edges, but may still be technically inside terrain (mountains, forest, water, etc), which is why it sometimes works and sometimes doesn't.
To workaround this bug, you could use the rod of recall near the top left of the map you're in - 2,4 and 3,4 are free tiles. In other words, stand 3 or 4 tiles away from the left of the map and 5 tiles from the top of the map.
