Bug on death

Whenever I get knocked down to 0 hits, my character disappears. I do not go to the Eidolon Plane, despite having lives left. The NPCs keep moving, the clock starts running very fast, and I can't do anything. Below are the error and traceback sections of my log; the full log is too big to attach.

Any thoughts?
