Today, I died because I thought an item that had an equippable ability was equipped and I clicked on it. I know it's useful to clear out old abilities that belong to items, but sometimes you forget, and just equip an item rather than doing damage.
I don't feel it would be very hard to code, and a great QoL change. All it would mean is that just as an equippable ability gets placed on the ability bar when you first equip the item (or get the item in main inventory, not sure which), so it would be automatically deleted from your ability bar when you unequip it (and then put back when/if you equip it again).
However, item abilities that can be used without being equipped would still remain on your ability bar for all the time it is in your main inventory. Of course, it would be useful and very much appreciated to add such active abilities that don't require equipping to the bar automatically, and remove such abilities from the bar if you transmogrify or drop the item.

Well, I was already working on something similar, so I tweaked it; does this cover your requested use case?

Yes, but I feel it would make a great QoL addition for the main game too. I would tend to think that a number of deaths would be accounted for by wielding an unwielded item instead of doing damage, a shield or something else to prevent from dying.
