10 days ago: Character had blood of life and died at level 20 (not to be confused with the level 24 death to turn into a lich, this was a death at level 20 as a doomelf) the blood of life was not quaffed but I was given the option to ressurect and continue the playthrough.

today: Character had blood of life and died at level 21 no items that cause the wearer to be treated as undead. Blood of life was once again not quaffed but no option to ressurect was given.
Both games were on roguelike setting. It is possible I was resurrected using some other mechanism and I am misremembering being given the popup with the option to resurrect but I am pretty sure this is the character that had the blood of life.

You do need to drink it before dying.

It never works if you do not drink it.
It always works when you drink it except in two circumstances (as far as I am aware):
It checks if you are undead when you drink it. If you are then it is wasted and you get a message in the combat log that nothing happens. Being considered undead from an equipped item will do this as well.
Once you drink it (successfully) it will show on your character sheet. The character gains a flag that is checked when the final death dialog is displayed.
When you die and the final death screen is generated the option to resurrect with the blood of life appears if the character had the flag. This will not be an option if you are undead, including wearing an item that considers you so.
There is absolutely no circumstance where you can resurrect with the blood of life without having consumed it first. There was certainly something else responsible for you being able to resurrect in this case.
