Game Killed Me on the Eidolon Plane

I was in the high peak and I got hit by a skill that "removed me from the timeline", followed by another one that did 1500 damage. I died, and then this happened (see screenshot) and I was "utterly destroyed" with two lives remaining. Definitely a frustrating way to lose many hours of work. I wish I had copied the game log before I returned to the main menu, but I wasn't thinking at the time.
