Eksatin's Ultimatum

The description says "decapitates a weakened target on crit". What does "weakened" mean here? Below 100% HP? Some specific debuff status? Or something else ?

<25% life (<10% of rank 4, which I think is just bosses). Also, I think it can be saved against.
