a new difficulity that combines NE and MAD

I just tried MADNESS and get my alchemist to lv 30.
Fighting a hell lot of rares, uniques, and bosses are really fun, but being one shoted is not.
So I would like to have a difficulity that combines Madness' rare rate and NE's level scaling.

Or maybe a new game mode option that changes the rare rate, and you can combine it with any difficulity?

There are some simple addons that change the rare rate, or else add random uniques instead of rares.

My favourite add on, by far, is the one that changes every (non-vaulted) enemy into a Boss.

There are also add one that affect character progression, if I'm not mistaken.
