So I've fully explored the "lost wife" dream area. Killed everything, which were all ghouls. No wife.
So it seems I'm just stuck now.

The canon answer seems to be "punch yourself to death and try again".

I've gone past that part now.

Though I suppose I should still be able to go back to the Caldera and use the altar to kick in another dream, right?

If you use the Altar, and you die in the dream, then you die for real.
