I think most people that have played for a while have accidentally tapped Z and had their escort near-instantly die. And while I'm not for the game playing for you, this is one of those spots where you can tap a button and get screwed over throughout an entire dungeon sequence-basically, it's less something intense where you need to be paying attention at all times, and more "Don't press that button you may well press all the time, while doing an entire dungeon floor". Easy to screw up out of habit.
I don't actually remember the last time it happened to myself, honestly(few runs ago and I think I stopped it; The benefits of a slow computer there.), but hearing some friends comment, it does seem like something that would be nice to have a little extra prompt for, if possible.

No extra prompt, just disable autoexplore completely while escorting (like it's done in the Sandworm lair, for instance.) Basically what Earwicker said; just disable it completely, there is absolutely no reason anyone would want to auto-explore with an escort, ever.

However, please do not do it as in the sandworm lair. Indeed,hitting Z there makes your character rest, then issue an error message regarding the fact that autoexplore is disabled.
To me, the problem comes from the fact that the escort wanders away from I rest, not when I do explore. In some situations, I want to rest( e.g., when I make sure my escort is stuck close to me and cannot go anywhere).

I don't know anyone who would answer yes to that prompt. Perhaps just a message in the bottom saying "this lemming needs to be babied".
Then, if they need to rest, and the escort is being ridiculous, you can either hasten it's departure, or block his path. Instead of accidentally letting that loremaster wander into Wrathroot while you were resting.
