[1.0.4] Lost two lives to a death

Was killed by an adventurer, heard the death music noise double up, turns out I lost two lives.

1.0.4 Higher Reaver against a sun paladin adventurer.

Attached is a screenshot of the log.

Edit: I chose to return to the battle and appeared right back in the eidolon plane. Leaving the second time worked. Feels like some damage (the paladin's retribution?) affected me after death and killed me as I entered the eidolon plane.

Yeah, not the first time it's happened to me, hoping that I can provide SOME information that might help. Was hoping it had been fixed, seems not so.
