This is getting exasperating. I've now had three game winners who didn't register as such in the vault.
In all cases the online server went down while I was playing, resulting in it not recording the win.
1) Is there a way to fix this?
2) If not, is there a way to make sure that it doesn't happen, or add some sort of warning about finishing when the server is down?

I've had this problem a lot. What I"ve discovered is several things:
1. When you first start the game (i.e., before you even load a character), wait about 3 minutes, and this often establishes a good connection.
2. There is always a way to know if you are being updated in the vault, and that is, if you see the option, Link item in vault. You can get chat, but not this option, and then the char won't update. I have no idea why this is the case, but if Link item is available, then the vault always updates.
For a more advanced way of ensuring connection is working, create a shortcut, and change properties to ...\t-engine4-windows-1.0.1\t-engine.exe --flush-stdout
Then use a tail.exe program to watch the log in real time. This way, you can see when you have vault connection by watching log, and then start playing.
The only downside is it is slightly slower to play this way on some computers. I"m assuming of course you are on windows.

Is there a way to get a character to update once it's been turned off?

Don't think so. If you are online, you could try loading the character if it still exists.
