Those don't look like nightmare achievements, they don't say (Nightmare) after them... Are you also playing roguelike? If not, you can't earn the nightmare version of achievements (same as insane, sadly. Which sucks for us adventurer players.)

That seems like a crazy restriction. The one death I incurred in my entire time doing that was completely unavoidable, a random manathrust crit from the boss. But, I'll give it a go on Nightmare Roguelike....

It is a crazy restriction, for crazy people's crazy achievements. 
Kinda would like to see Roguelike give separate achievements from standard ones, and Nightmare-Roguelike and Insane-Roguelike get different achievements from Adventure Nightmare/Insane, myself.
