When you die, one of the options on the permadeath screen is to restart the same character. The addons after the restart character aren't the same as the old one, but are your currently set addons.
This can be a problem if you are turning on specific addons on and off for specific character types - for example, if you have multiple class addons that you are swapping between to avoid superload conflicts or RAM usage bloat, you end up with a character of a nonexistent class, using nonexistent talents.
