I have 12 % Fatigue without anything. The only unusual thing I have done was adding and removing generic skills points many times.

I might start another thread about this, but I noticed in b42 if you wear an item that is "immune to teleportation" and you teleport, the item falls off as expected. However, at least some of the time, your character keeps the stats that the item had. I noticed this when I wore a rare charm that gave telepathy to demons but was teleport immune. I maintained the telepathy after the item fell off following a teleport.
Also, I think constantly teleporting and re-equipping those types of items can sometimes duplicate them. Unfortunately I don't have exact instructions on how to reproduce that bug.
So, is it possible that you work a rare teleport-immune armor with 12% fatigue and it fell off?
