Last time I have lost a character of about lvl 25 with 500 hp who got instagibbed by a skeleton archer while autoexploring. I have caught a critical arrow of 255dmg and somehow it dealt damage twice, though I have failed to understand why. That was a regular master skeleton archer of about the same level as me, maybe a little higher. Skeleton archers seem to deal far more damage in general than most other similar enemies, maybe it should be fixed?

Being about the same level a melee skeleton fighter hit me with critical Overpower ability for 1200 dmg and one-shot-killed me even under full protection of a decent damage shield and with full hp. I sort of got used to melee skeletons dealing extreme amounts of damage - at least they can be avoided by not letting them get close.

Stone Touch has insta-killed me a couple of times. The latest was a corruptor classed as an alchemist. Stoned me (from ~5 tiles away), then blasted me with blight damage. This was essentially a 5 turn old-style stun during which I couldn't do anything but take full damage from whatever was attacking me.

A level 30 Naga myrmidon @ lvl 1 Daikara instagibbed my lvl 15 Archmage (220 hp) a couple of times...

All kinds of Doomed randelites are annoying.
In b42 Solipsists are even more lethal, they keep sleeping me and finally I got buried in the sand of the Sandworm Lair (Wild infusion? I used it and got sleeped again).

What's more, I think in insane difficulty, no randelites should appear in the first dungeon or Norgos' Lair, which is an easier dungeon to start with in insane difficulty. Lots of long corridors where can be used to kite or regenate. There're so many things can kill a lvl.1 character in one shot, like corruptors, archers, doomed, etc.

While part of the death was to inexperience and not running away fast enough it still should not have happened. I got my skeleton bulwark destroyed by a level 15 ghoulking that spawned on level 2 of the blighted ruins, what killed me was the 20+ turns of 22 hp/turn disease thing he hit me with.

That's just a regular elite. It sounds like you opened a vault, despite the game's warning about doing such a thing. Maybe the warning against opening vault doors could be a little stronger?

It was just a regular elite but the problem was that it was in one of those 3x3 rooms with coins on the floor that are exposed without protections to the rest of the dungeon so that I was just going down a corridor and then I'm being attacked by a bunch of level 2's and a level 15 ghoulking.

Yeah, I've noticed that those rooms seem to lack the protection against out of depth enemies that tier 1 dungeons normally have, like vaults do.

Unlike vaults, though, they're open air.

The biggest problem with it that I only realised today is that the stairs can be in one of them, consequently I could have gone from floor 3 to floor 2 and had a level 15 elite next to my level 2 char with no possible warning or time to prepare which would have been even worse than what did happen.

Something new happpend which was so bad that it might even be a bug. What happened was that I started the ID and the first room was adjacent to a vault, I killed the few creeps in the room and then started getting attacked Dreadmaster that I couldn't see, I think that it escaped from the vault as I've noticed previously that if you teleport in a vault it teleports you out if possible so I think it teleported itself out.

Sort of one-shotted by a level 35 armored skeleton warrior berserker rare. I think what happened is that it used Disarm, which gave me the -15% global speed cross-tier effect, enough to allow it to act again before I could react, followed by level 9.6 Death Dance for a combined ~750 damage at a point when my max health was about 550.

That's a rush daze in normal bounds, then 260 so damage on skullcracker there

Reminds me of losing a character to skullcracker vs. an elite with the Steel Helm of Garkul.

Perhaps skullcracker, and specifically helmet damage and skullcracker are the culprit?

Today I was instagibbed by 600+ damage from a firetrap that was triggered by my necromancer's summons multiple times while he was resting. I did not even understand what happened until I checked the game log.

I've lost more than one necromancer this way.

I really, really want the delayed explosion fire balls to stop resting and auto explore if you can see them. Not as important, but the boulder trap projectile should do the same thing as well.
