After returning, my character was cloned.

Hum any other things happened at th same time ?
Just dying I cant seem to reproduce it

Exact same thing happened to me as well - died in the daikara, and after coming back, there was a clone of me where the precognition began. I think it might have to do with dying from special effects, e.g., flame damage from burning, or certain curses.

After it happened I tried to look in the game log to find something to paste here, but I couldn't really figure out what to look for. When it happened to me I think I was killed by a skeleton warrior in a vault in Dreadfell.

Same thing happened to me. I had fire dot damage on eidolon plane. Though I didn't penalised the life.
Precognition has some cloning problems even when the character does not die, it seems. I used precognition for map revealing purpose and clicked the map madly even when timeframe was being reloading- and now a clone came out of nowhere. Quite frequent.
