I would recommend that you get a new copy of TOME and see if that fixes those issues at all (I believe they are very unusual). Sorry I can't be of more help.
For the record, these are the only pieces of lore I have ever gotten like that in some 15-20 characters. And I remember reading in the "make a vault" thread that someone said to make 30x30 size dungeons or less because the bandit fortress 35x35 size could cause problems. I think this is what has happened here, and I think that this particular vault should be made smaller as a result.
Keep in mind that when I say the lore wasn't there on the bandit dungeons, it wasn't there ANYWHERE. Not even in the vault or inside the vault's walls. It just wasn't there. I think this is because I got an unlikely placement close to the center of the map on both bandit fortresses, because there was only a one-wide walkway around both of them. I think the lore placement algorithm said "welp, no place to put the lore with all these trees" and gave up.
Also, I don't think it's my copy, because I got another bandit fortress since then that did spawn the lore properly, as the placement of the fortress was on the bottom of the screen.
The one Trollmire lore inside a forest I had to pickaxe to was weird. Never seen that before or since. I know you had to hack to pickaxe to it because autoexplore didn't find it (and I double-checked to make sure). I've seen other people post that they couldn't find the lore in the Trollmire before so I suspect this is just a rare bug that's been around a while but doesn't show up very often. (There's a kind of similar bug where air elementals occasionally show up outside Derth proper on the Derth quest, because there's a one-space diagonal opening in the lower-right corner of Derth. At least you could see the enemy there, though.)
And about the lava lore...well, I'm betting the game just doesn't check lava when it picks lore spots, because it's a rare tile type. I'm pretty sure I've found lore in vaults several times so I don't think that's a problem.
