amulet of teleportation uses amulet icon and not teleport icon in hotkey bar

All items use their own icon in the bar, last time I used talent icons. Don't think this has changed.

Some things are getting the wrong icon. My dwarven-steel pickaxe of wreckage is using an icon of an arrow through a heart, which I think is the icon for Precise Strikes. Should I post the save game?

I know that disengagement items use the curved arrow symbol instead of the item.
