Some of detrimental effects a new to me - for example what "overbalanced" mean? I didn't get it, only inflict, so I have no idea what does it do. Are there list of new(or not so new) effects which are not in wiki?
About cross tier effect of overcoming defense - only attack count vs defense, damage doesn't count vs anything? Or vs armor ?
About attack - only dex affect attack or str too?

Check out the new advanced Tutorial if it works for you, the last part explains it.
But here are the facts ( as far as I know ):
Cross-Tier effects are caused by having a high attack value vs low enemy defense value.
The attack values are physical-,mental-,spellpower.
The defense values are physical-,mental-,spellsave.
The physical cross-tier effect is called Off-Balance: -20% (move-?)speed and criticals on it have 25% more physical power
The mental cross-tier effect is called Mindlocked: Sets one talents on cooldown and prevents talents from cooling down.
The magic cross-tier effect is called Spellsomething: Reduces resistances by 20%.
Example:
Bellowing Roar uses your physical power and checks it against the enemies mental save. If your physical power is far higher ( don't know how much higher it has to be ) than their mental save they get the mental cross-tier effect Mindlocked.
In addition to that normal attacks get a Accuracy vs Defense check on critical hits and that cause Off-Balance, too.

So do attacks that don't carry a save check [like a straight up attack talent or bump] - they don't generate a check for a cross tier effect?

Except unless specifically noted (like archmage's Spellcraft sustain) yes
