Were there any unique undead left?
There are some skills that can kill you when the talent's damage makes your HP drop below a certain percent. I suppose that would not bother with Blurred Mortality.
One example is Quantum Spike, which kills you if it makes your HP drop below 20%. Perhaps one unique had access to one of those talents.
No death message sounds odd though, but it's not the first time I have heard that the last messages before death are missing.

EDIT: there's also a weapon curse that has a 3% chance on hit to instakill "weaker" monsters

I just had the exact same bug. I "died" with 400+ health to a single near-death Dread. Something HAS to be broken there, intended or otherwise. It was in the Ruins/Tower of Telmur, if that helps.

Aussiemon did you save the log? te4_log?

Generally when you encounter a bug like this we'd need as much information as we can get to track it down a long with a snippet of the log if possible (just the area of the log where whatever occurred occurred).

For instance are you referring to Blurred Mortality? Did you have this talent active? Did it get dispelled? Did you have an effect that artificially inflated your life that got dispelled?

Saying you got killed by a dread while you had 400 hit points doesn't really help much :?

In the future if you could try to give as much information as possible it would help out a lot :)

Thank for the advice on reporting bugs! I really like this game and would like to see it go far.

Back on topic, where can I find/save the log? I'm afraid I ragequit (sorry!) when I was killed by the presumed bug. The character in question had no talents deactivate from the Dread, I think only the Dreadmasters can do that. My character was also a Dwarf Temporal Warden, level 36. I believe the last thing I did was just a regular attack on the invisible Dread.

I had noticed something strange while I was surrounded before this: creatures such as storm drake hatchlings and rogues were walking into wall tiles to attack me, despite not having any kind of ability to do so. This may only have been after using the Rune of the Rift rune, as that is the only time I specifically picked up on it. I don't know if the two are related somehow.

Finally, I know for a fact it wasn't the Spirit of Whatshisname in that dungeon who killed me, because the Dread was the only one there upon return and departure. I waited after coming back and he never sought me out, so I'm relatively sure he wasn't there. In the interest of full disclosure, my character is probably too powerful to be hit that hard by a Dread, critical or no. He had numerous buffs and was rather godly, if I do say so myself.

Hope I covered everything! If I can find the log, I'd be more than happy to upload it for you! Otherwise, I'll just see if it happens again and upload the new log.

Edit: Fixed typo

The log is in the main game directory, and is called te4_log.txt. However it gets rewritten every time you start the engine up, so unless you haven't touched the game since then it's likely lost.

I remember that I was torn apart at a molecular level.
Still, there was no message of me getting killed in the log, so something needs to get fixed.

So instantkill under a certain % of life are the weakness of Blurred Mortality, just like increased hate gain are an advantage ( that is useless as the moment ).

Hmm, that shouldn't happen. That's the insta-kill message for Quantum Spike, which shouldn't work on players.
Sorry about all the parentheses (sometimes I like to clarify things).

Maybe it does some sort of HP < 0 check that always triggers that (just to get the special message when it kills), and it's now getting confused by blurred mortality?

It does, though. I got Quantum spiked to death, ignoring damage shield, while my HP was below 20%, with the same message (and no damage in the log) in b31 or b32
