Yea I did it many times before. Tried relaunching the game, tried different saves and everything.

I get the prompt like before it's just that I don't get any achievements, and I should get loads since I never came this far with this difficulty before. I do like this feature that you automatically get the easier versions of an achievement if you get the hard one, it removes the necessity to beat the game over and over again just to get it on every difficulty and death mode. Just too bad this feature isn't automatically built into Steam.

Anyway, I know this is an old game now, are the developers still working on it and fixing bugs like this?

I was not aware of this but it you need to launch ToME through Steam for the achievement push to work. Tested it myself just now and works fine

As far as devs, yes developer (DarkGod) is working on a DLC (Lost Land) currently, tho due to IRL problems it's being heavily delayed

Yea, I know that. That's what I was doing before and it always worked, but last 3 days it doesn't work for me anymore. I'm not sure if it's related, but I got some lua error messages after I went back into the Last Hope Graveyard since having faced the boss once there and died to her (playing on adventure mode). When I went back there I got error pop-ups every single time I moved, so it felt unplayable and I ended up just leaving. The game works fine otherwise still, I guess. But since then the pushing achievements from te4.org doesn't work. And it doesn't work on other saves either or beginning a new game. If it works for everyone but me there must be a bug with my account. Maybe I will try to re-install the game or something.
