[1.5.10] Dark Crypt Necromancer spawned two Doomed Shades

As in the title: When the necromancer of the Dark Crypt spawned a copy of me, he made two. My only evidence is this image:

If you take a look at the minimap in the top right corner, you'll see I died on the bottom left and that there are two purple dots, each a copy of me. They were absolutely indistinguishable in the logs, too - neither of them were a Forgery of Haze.

The only thing I can think of that would have caused this was the fact that I opened the fight against the necromancer with Out of Time, before he had made any copies. Maybe that bugged him out?
