There's supposed to be another floor switch to let us out of the trap but it isn't working

Assuming you haven't already, you can get your save fixed by a mod if one is in chat, or by zipping up your save and getting it to Stunt as described here: http://forums.te4.org/viewtopic.php?f=38&t=45222

You sure it isn't on the left side of that room?

Weirdly, I had two of these vaults in the same game. The first one was fine, like you say there was a secret door on the left. The second one no such luck.

I got stuck in that vault the same at Embers release and pretty sure there wasn't a switch there. At least I could still recall out of it at the time :p

Its not a visible switch, it just opens when you go near it.
Maybe if an enemy walks there first it deletes the trigger without opening it?

I got "stuck" in this vault during my embers playthrough(I was able to escape by spamming dream walk luckily). Pretty sure there's some kind of issue because I hugged every wall beforehand.

I've never gotten out of that vault without a teleport
