Demon Form and the Puce Box

In both 1.3.1 and 1.4, I've noticed that whenever a character is sustaining the Demon Form Corrupter sustain, their sprite is replaced with an over-sized puce box, except for a few short moments where the character and effect are visible. This has affected both my player character and... that demoness I can never spell. The one with too many s's and apostrophes.

Demon Form ? You mean Flames of Urh'Rok ?

Yes, that. I'll have to make a save specifically for this, so it may take me a bit. I'll also include some screenshots when I can.

Actually, I had. The last thing I did before saving and quitting was to use Flames. I will give it another go this evening; I will activate it and wander around for a bit.

Well, I made a new save. The Puce Box happened a few times while I was playing.

Fayd, while you're waiting for DG to answer, you might want to play around with shader options under graphics.
"Boxes" of various sort are usually your GFX card not liking certain shaders.

...I'll admit that I didn't even know those options existed.
And I even did think it was something up with a shader or particle system.
