Character reverts to previous incarnation.

My character is having some really strange issues. After exploring a large portion of the world map my character goes to town. When he goes back to overworld map everything is unexplored again. Also when entering same town again it has new seed. There are new NPCs and shop inventories are different. You can do this multiple times and everything is always different.
After I saved in Last Hope and loaded the game to my surprise my character wasn´t there but in Trollmire 3 instead where this character has never been. This level is identical to Trollmire 3 where my character died previously. I also had my old characters equipments. I intentionally got killed by same monster that killed my old character.
I made new character with a same name and immediately saved. After loading I was in Trollmire 3 again.
I have many characters in the character vault with a same name and it is probably causing this issue. I should just do some cleanup but I am reporting this bug regardless.
