I cast it, I get a beam targeting thingie (i.e., target area 1).
If I hit escape OR if I click somewhere, it changes to a 3x3 grid target centered on my cursor. I then have to click again to actually activate the teleport.

That's not a bug. Phase Door at effective lv 5+ lets you choose both who (the 'beam' targeting) to teleport and where you want to send them (the 3x3 grid).
