ToME: the Tales of Maj'Eyal

I was just playing my stone warden, and I hit lvl 14, and I had 0 deaths, so naturally I decided to go to Derth and take on the storm. I cleared two groups of storm elementals with relative ease, however I came across a particular mob that instantly one shot me. According to my chat log it's named Ultimate Gwelgoroth. Now being one-shot was okay, I still had 3 lives left. However upon taking my first step in the Eidolon plane I got one-shot again while still in the Eidolon plane? So one death that may or may not have been legitimate, and one buggy death that shouldn't be possible. However the real problem is that upon dying in the eidolon plane I received the utterly destroyed achievement, and lost my character. I don't know if I can ever get that character back (it's only lvl 14 after all, not too big a deal, I'll miss my gear though) but I feel this should at least be fixed for future play-throughs. This is the first time I've experienced this bug.
