During the ambush after dreadfell, if you have the lava ground from the crown of burning pain over the area where the exit portal should spawn, it doesn't (careful not to save or character is stuck).

If you don't do the sandworm lair before heading east, when you return there's no way to get the special boss, so the eye of atmathon isn't guaranteed.

An artifact +2 tentacle wand is identical to a regular +1 tentacle wand but with twice the cooldown. I was pretty excited to find one, but then my spirit was crushed.

Not a bug. Though, really, you should be killing her anyway. She's quite tasty and provides a highly useful boost.
