[RC3] Precognition doesn't fully reset universe

While using precognition to loot the LH mausoleum, I encountered a few instances where things did not reset to the way they were before precognition.
This occurred with two different characters, both in exploration mode.
My Paradox Mage met an Arcane Blade skeleton who once managed to freeze her after precognition ended, and once left a flame projectile behind after precognition ended. (Two separate instances of something being left over.)
Hypothesis: Projectiles are not cleaned up by the reset.
My Temporal Warden died to some of the undead there. After resurrecting, the undead were gone, but so was a clone of my character. And I wasn't able to control myself for quite some time. Occasionally one of us would cast a sustained spell. I couldn't tell whether it was "me" or "the clone." Eventually (not sure how many turns because of the lack of control, but it seemed like a long time. 2 or 3 real-world minutes), the clone disappeared and I regained control.
I had him die again, to test. Again the clone appeared, but this time I had full control. Being myself, I killed the clone. And gained experience for it. However, the clone went back to full health. I tried to continue attacking it, but now every time I tried to target the clone, my cursor jumped to my character. Casting my AoE spell so it hit the clone but not me hurt both of us, and healing myself healed both of us. So I killed him. We both died. This time, when I resurrected, everything was normal again.
Hypothesis: I have no clue.
Edit: Being the one I could test, I played around with getting projectiles to exist post-precognition. Here's what I found:
Playing as a Higher Paradox Mage, casting precognition, waiting until the spell cooldown showed 1, and then casting Turn Back The Clock could, under certain circumstances, cause the projectiles from the spell to remain after the reset. Those projectiles would not move, but they would deal damage + cause their effect if I moved onto one, or if the reset of precog caused me to move onto one.
In order to semi-reliably get the projectiles to remain after precog, I had to move an even number of times with a global speed of 100 and a move speed of 152, wait for the effect to be at 1, and then cast. Simply waiting with '5' did not work, nor did moving an odd number of times. They would only remain if I had moved 2, 4, 6, or 8 times during precog, and even then it was unreliable.

And then, after a whole lot of trial and error, I managed to kill myself, after coming out of precog, using projectiles that were preserved. I did this twice, after much casting. Both times, I was sent to the Eidolon plane, then immediately sent to the Eidolon plane (it looked like I teleported, but with an extra save step). After I talked to the Eidolon and asked to go back, I was sent to the first Eidolon plane. And then I had him send me back, too. Upon returning to the physical plane (for real this time), I had two return messages but only one death message.
I think, ultimately, all of this weirdness comes from the projectile bugs. I could kill myself consistently by having low HP then casting two Turn Back the Clock projectiles at myself while in Precognition. I'd test more, but I eventually managed to kill myself on the Eidolon plane and thus lost my progress. Did not know that dying there would be a permadeath even on Exploration mode.
