So, the achievement for saving all the Derthlings is likely buggy. Two people are listed as having achieved it, and the one that's not me is 't8' which is a testing account (i think).
I've attempted it several times, counting very carefully, but the only way it seems to work is if the agent of the arena is either saved outright or never spawns in the first place. This implies to me that his wandering off after the arena quest is being counted as a death.
A much less likely explanation might involve Derthlings getting spawned in the moat, I suppose.
While I have felt pretty cool indeed being the only one to get this, it's probably not intended to be this way. <grin>
