Unless you're doing it telekinetically!

It makes perfect sense to me. Your weapon gets knocked out of your hand, and you use your telekinesis to pick it off the ground and mash the enemy with it.

... not really, no. There's... (still? thought this was fixed betas ago) a typo in the description of Telekinetic Smash. You're smashing stuff with your telekinetically wielded weapon, only striking with your mainhand if you've got Conduit up. Disarm is only batting your actually-wielded weapon away, not your telekinetically wielded one.
