[svn2427] Phase Door versus Teleport scaling

So it looks like PD got a bit of safety boost in terms of fizzling that didn't get ported to Teleport, and the order of target actor/area is switched in the two talents as well. Since all of the numeric functions are in doRange and doRadius, maybe the action function from PD should be copied and then updated for Teleport?

Unless it has a min range and you're in a very confined level...
