Some monsters have "smart" castings. I suppose the thunderers don't cast lightning bolts but lightning strokes. Same for orc pyromancers using Fireflash.

They target the X spot and the effect still affects you since you're in the radius of blast. Thunderers aim at the same spot and the lightning stroke bounces on the wall and hits you.
