Does Underwater Cave always appear?

I've just reached the Far East for only the second time, so I'm pretty excited. 
Finished Ardhungol and went in search of the Underwater Cave before trying the Vor Armoury, but I can't find the Underwater Cave anywhere. I've looked along the entire coast of the Far East. Does it not spawn in all games? That's where I died last time I got to the Far East, so I was hoping to get through it this time. Any ideas?
