How Do You Meet A (Spoilers)?

I know there's a way to meet a surviving Sher'Tul (or is it time travel to when they were alive?) but I've never been able to do it. So, how is it done?

This is technically spoilers, but whatever. There's a chance when you use the Exploratory Farportal you'll end up in a small event rather than the usual randomized dungeon. That's where you can meet a Sher'Tul and get the "A living one!" achievement.
