Effects on melee hit

"15% to daze" and "slows global speed by 30%"
how do they work?
on every MY hit?
or when I'M hit?
do same effects stack from different items ie two "slows"?

If it's "On melee hit", it's every time you hit the enemy.
If it's "When hit in melee", it's every time you get hit.
I don't know how different slows stack, but different effects will always stack.
And yes, this means abilities like Flurry that hit 12 times have 12 chances to daze and slow. Powerful builds use a lot of procs like that.

What I want to know is what qualifies as a proc on hit for the purposes of the staff and shield accuracy bonuses. Burst on hit/crit doesn't seem to qualify. Percentage daze or blind or damage reduction don't do damage. I've never seen anything else on staves and most shields don't have anything either.
