Help with a death

After clearing out Dreadfell with my lvl 20 bulwark dwarf. I was ambushed outside the tower with orcs who wanted the artifact I looted from the necromancer master.
At first I was overwhelmed with the enemies because I wasn't focused and was caught off-guard . But I handled the situation pretty well and almost survived the assault.
The thing I don't understand is my teleportation rune didn't work. And I made a foolish mistake thinking it didn't work because of my "last stand" talent (which disables movement) that was active and I turned that talent off decreasing my hitpoints drastically and tried to teleport away But still I couldn't teleport!! The teleport rune just went on cooldown and nothing happened. And then I died horribly against those few remaining archer orcs :).

The thing I want to learn is what caused my inability to use the teleport rune. I looked at the log. And It just said my charcter uses the teleportation rune and continued with the damage reports without saying if and why it failed. Does anybody have an idea why this could happen?

PS: I think there can be pop-up message saying that the teleportation failed because of this and this reason. Or It can just be written in the logs.

Most likely it's because your teleport has minimum distance and the map in question is pretty small so there's nowhere to place you.

If you die on that quest after killing the boss you will lose a life but if you die while the boss is alive you don't.

Hmm thanks. If that is the case I think it should be considered as a bug. The rune should take me as far away as the area map allows.

By the way the teleport rune had a range of 64 with a minimum distance of 15.

And yes I killed the boss there =(

That's part of the tradeoff between Teleport and Phase Door. With PD, you have a set range between 5 and 15, depending on the one you get. It will work in the small spaces, but won't send you as far. Teleport sends you far away, but fails in the small maps and can land you in more danger if you haven't cleared most of the map yet. The one to look for is a Controlled Phase Door with a high distance number. You can target a radius location you want to land in, though it has a chance of failure if you try to go through a wall.
