Dumb newbie question, do you wait till you die to drink it or drink it whenever to get your extra life?

Drink it when you get it, unless you're undead(it does nothing in that case).

A ghoul can breath and retch, they should be able to accommodate the reverse and drink. That said, something called the blood of life should probably make them human before it gives them an additional life

I don't know how permanent that transformation would be, what with a Ghoul's propensity for rotting away as the go... Might be better for them to leave well enough alone. For their own sake.
