Is it worth the stat randomness to drink this?

Wyrm bile? Yeah you get a cat point for free, even if you don't have anything you want to unlock you can just spend it on a category.
I'm not quite sure how the stats randomness work with stats cap thought.

It is independant of stat caps. It you have, say con at 60/78, and you get 3 stats, you will have 63/81. So it is better to quaff the bile with the most important stats maxed, otherwise you have no way to go above 60.

There's more to the Wyrm Bile than the stats and category points (that are good on their own, assuming you're lucky with the random stats) , when you have previously eaten the Heart of the Sandworm Queen , after you drank the Wyrm Bile you become "Close to the Draconic World"
A condition required for 2 of the prodigies (Draconic Body and Draconic Will) in case you're not playing a Wyrmic

So regarding the stat randomness: The stats moved around are treated as points purchased with level-ups. Meaning that stats reduced can be repurchased up to max. If stats increased are already at max, the bile allows you to EXCEED the max. Given that we're talking a few points here, there literally is no reason not to drink it unless you waited until level 50 already (at which point you wouldn't be able to buy back reduced stats) and don't want to risk it.

The stat randomness ranges from -3 to +6, so on average you tend to come out ahead anyways.
