Just saw the first image in-game as well. Was on my 2nd far portal, so it looks like it's pretty random. Was a good scare btw as I suddenly saw my HP ticking down fast without being able to do anything about it --> especially as I'm playing Roguelike as a skeleton with Re-assemble still at just 4

Glad the scare worked :D It is indeed a random chance. Getting it first or second trip is pretty lucky.

I got it during the very first far portal I used. I just thought that's what you get by default the first time, heh.
