Something for ID

First, please correct me, if im wrong on some game mechanics.
AFAIK unique bosses and artifacts only spawn once in a game.
If for some reason you cant take them/ kill them, you need to come back later.
In ID this is impossible in most cases.
So to get any good gear, you need to do the vaults and this is close to impossible to some classes at early lvls and suicidal for everyone at some vaults.
But every boss and every artifact that spawns inside the vault is gone forever. And i think vaults contain one artifact with around 50% of the time.
Getting 3 vaults at ID 1, like i just did, means you permanently lose up to 3 atrifacts.
So the question is: Can you somehow reset the "appeared already" trigger for everything in unopened (!) vaults when leaving the level?
I tried taking on vaults only when i thought its no longer suicidal (one teleport and enough hp to not get oneshot by dragons etc.), but doing this results in beeing massively underequipped.
And as a secound thought.
We all know those enemies that should not appear already such as a lvl 25 Xorn but still happen sometimes in ID.
This can ruin a game. You are lucky if you make it to the stairs alive and you deffinetly lose the much needed loot/Xp again.
It may be impossible to cancel out those appearings, but maybe they could get some debuff like rares do under lvl 10 and 20 whenever an mob level exceeds 3 or 4 times the ID level?
