ToME: the Tales of Maj'Eyal

I have effects on my mindstars, like 20% chance to cause insanity (=gloom etc). When I cast Call of the Ooze as a oozemancer, they never apply, but they do if I simply bump attack an NPC.

Yet if I put on equipment that hits on melee, this seems to apply as an on proc.

So, can someone please clarify how this works?
