Adding Custom Portraits

Long question in short form 
How do I go about making and then importing my own custom portraits into ToME?
Please assist if you can...
1. What image dimensions?
2. Where do the completed images go... in which specific directory folder?
3. Is there any special considerations needed with "ini" files or the like?
4. Any further assistance that is offered would be greatly appreciated
