Corrupted Sand Wyrm disappeared

Playing PM, went to fight sand wyrm again, paradox got high, anomaly happened, corrupted sand wyrm disappeared and I couldn't find it anywhere. Re-loaded from when I entered the dungeon so it's fine, but still annoying.
