So my archer went down to see Tol Falas. He was hit by a ghoul king he couldn't see. Still not sure whether he was buggy then or I just missed him. Either way, just to be careful, since I was still on the stairs, I went up to the world map. Planned to come back when there were definitely not invisible ghoul kings. And then he followed me onto the world map. He wasn't visible there but :

actually, he did not follow you, you just got unlucky and got diseased when he hit you.

right now diseases do not stop you from going to the world map, but you cant use items on the world map either, so it does need some work on it. (IOW, it is a "bug", but not what you said) (a "bug" in the sense that it should at least let you use some items on the world map or stop/warn you if you try to leave a zone when diseased)

It may not be clear when I left the dungeon, but he kept hitting me on the overhead map as I walked along. I kept being told the ghoulking was hitting me, at least.

That could be semantic, and a sort of bug. I've noticed that when I get poisoned by, say, a rattlesnake, even after I killed the reptile I still get messages saying "the rattlesnake hits" or somesuch when the poison causes damage.

It could be clearer.

Fair. I've seen that too. Didn't realize that was the case because I didn't know I had a disease, but that probably is what I was seeing. Still, I shouldn't be able to leave the dungeon with a disease if I can't enter a dungeon with a disease. Particularly when one can't cure the disease on the world map.

yeah, I would like it so that you can leave dungeons when poisoned/diseased, but then make you able to use certain items (not teleport or phase door or similar ones).

then make it so you cant flee when there is a monster adjacent (unless your speed > their speed) (not generally the case)

This is actually a real bug. My last char saw a Ghast summon a Ghoul on top of himself! And the monster is actually able to hit the character and follow him through stairs. The only way to fix that and to avoid being killed is to move away from the stairs... then the monster will follow the character and become visible (and killable) again.
