Was in the middle of the "Storming the City" quest on my Normal/Exploration Dwarf Alchemist save file and after being killed by Urkis and being teleported outside the area so I wouldn't die again I went to Derth. Every action I took inside the city that required a turn, walking, using abilities, brought up the Error window as well as when I was inside Angolwen. I do not get the error while traveling on the world map.

Managed to figure out what I'd done that caused this, I went back through Angolwen and teleported back to the peaks, the error kept popping up with every move till I found Urkis and suicided into him, resurrected, rinse and repeat a few times and the error eventually stopped popping up with every action. Something about the game doesn't like it when you resurrect elsewhere when killed by Urkis.

I suspect you got hit with his Hurricane and it didn't clear correctly once you died. It damages you each turn normally. That would explain the error every turn, anyway.
