I've saved Melinda and have had her portal appear in Sher'Tul Fortress but it seemed that she disappeared with her portal with her.
Also, I paid energy to change the butler into a maid but then he/she/it reverted back too.
Also (2), should "Melinda, lucky girl" remain in the quest log?
My loyal kitty is still there though and everything else that was unlocked was there too so it I'm not sure if it is a zone reset or just a downright bug ^^

Sounds a lot like your progress wasn't saved. Are the changed things the ones you did the last or was there something that you did after them and was saved?

Well, after saving Melinda and going on the beach date, successfully defending her enough until she goes supernova, asked the butler about her condition, talked to her about living in the fortress ( albeit temporarily, for treatment 
), going kissy-smooch-kissy on her, then asking the butler to change to a maid... After I did Dreadfell and killing the "Master", TPed to the fortress as I've made the Rod of Recall do that instead, then noticed that her portal and her person is gone... 
I've since progressed the story and have gone to the East.
